I’m Interested in Starting Therapy
My practice is open to new clients. I see clients through a HIPAA-compliant secure telemedicine service. Use the form on this page to tell me a little bit about what is bringing you to therapy. Before I schedule a first session with a potential client, we have a 15-20 minute phone call where I’ll answer any questions you may have, I’ll ask a few questions to make sure my services are a good fit for your needs, and if everything seems like a good fit for we’ll schedule the first appointment, usually within a week or two.
What happens in a therapy intake process?
- A 15-20 minute phone call.
- You fill out four forms. 1) Consent for Treatment, 2) Telemedicine Consent Form, 3) Payment Contract, and 4) Face Sheet (this has basic information like you address, mailing address, phone number, and emergency contact).
- Then we have your first session
